ITANAGAR, Jun 3:  In a landmark achievement for the medical community of Arunachal Pradesh, the Tomo Riba Institute of Health and Medical Sciences (TRIHMS) successfully performed its first primary angioplasty, saving the life of a 61-year-old diabetic male who had suffered a severe heart attack.
The patient, visiting Arunachal Pradesh to see his son serving in the Indo-Tibetan Border Police (ITBP), experienced chest pain in the evening and was rushed to RKM Hospital. He was diagnosed with an inferior wall myocardial infarction (heart attack) and subsequently referred to TRIHMS for advanced care.
Upon arrival at TRIHMS, the patient, being within the critical window period, was immediately thrombolysed with Streptokinase (STK). However, he developed cardiogenic shock, prompting an urgent transfer to the catheterization (cath) lab for a primary Percutaneous Trans Coronary Angioplasty (PTCA).
The cardiology team, led by Dr. Rinchin Dorjee Megeji (Associate Professor, Cardiology), swiftly initiated a series of life-saving interventions. A Temporary Pacemaker Insertion (TPI) was performed first, followed by a Coronary Angiography (CAG) of the Left Coronary Artery (LCA), which revealed significant disease in the Left Anterior Descending (LAD) artery.
During the procedure, the patient suffered a cardiac arrest due to Ventricular Tachycardia (VT). Immediate defibrillator shocks and Cardiopulmonary Resuscitation (CPR) were administered. The Right Coronary Artery (RCA) was then hooked, and CAG showed a near-total occlusion of the proximal RCA. Balloon angioplasty followed by stenting of the proximal RCA was carried out. Despite another episode of VT, the team successfully stabilized the patient after repeated defibrillator shocks.
He was then shifted to the Intensive Coronary Care Unit (ICCU) for monitoring and recuperation. The entire cardiac interventional procedure, which began at midnight was completed by 1 AM on June 2, last.
This successful procedure marks a significant milestone in the history of TRIHMS and the state of Arunachal Pradesh. The dedicated cardiology team included Dr. Tony Ete (Assistant Professor, Cardiology), Dr. Sindu Damde (Junior Resident, Cardiology), Cath lab technicians Margo Bagra and Mukhtarul Ali, and nursing officers Karnu Poyom and Sandhya.
Dr. Rinchin Dorjee Megeji expressed pride in his team’s achievement, stating, "This historic success showcases our commitment to providing advanced cardiac care and saving lives in Arunachal Pradesh."
The successful primary angioplasty at TRIHMS represents a beacon of hope for advanced cardiac treatment within the state, ensuring that patients no longer need to travel far for life-saving procedures, stated a release.
